单心动周期实时三维超声对扩张型心肌病左心室收缩功能的评价 被引量:5

Evaluation of left ventricle systolic function in dilated cardiomyopathy by single cardiac cycle real-time three-dimensional echocardiography

摘要 目的应用单心动周期实时三维超声(sRT-3DE)定量评价扩张型心肌病(DCM)患者左心室收缩功能。方法 DCM组和健康对照组各50例,获取sRT-3DE条件下的左室收缩末期容积(LVESV)、舒张末期容积(LVEDV)和射血分数(LVEF),并与二维超声Simpson法测值比较。结果对照组二维超声与sRT-3DE左心功能测值差异无统计学意义(P>0.05),二者相关性较好,DCM组sRT-3DE法测量的左心室LVEDV、LVESV高于二维超声测值,差异有统计学意义(P<0.05);sRT-3DE测量LVEF小于二维超声测值,差异有统计学意义(P<0.05),二维超声与sRT-3DE两种方式有一定相关性。结论 sRT-3DE可以获取真正的四维图像,精确地测量心腔容积,评价心脏功能;对心腔明显扩大变形者sRT-3DE测值优于二维超声。 Objective To evaluate the left ventricle systolic function in dilated cardiomyopathy (DCM) by single cardiac cycle real- time three dimensional echocardiography (sRT -3DE). Methods Fifty patients with DCM and 50 healthy subjects were enrolled in this study. Their left ventricular end -systolic volume (LVESV), end -diastolic volume (LVEDV) and ejection fraction (LVEF) were obtained by sRT -3DE, and the results were compared with the parameters got by two - dimensional echocardiography(2DE). Results There was no significant difference of left ventricular function between the 2DE and sRT - 3DE in control group( P 〉 0.05 ), there was good correlation between the two methods. While the LVEDV and LVESV obtaind by sRT- 3DE were higher than those by 2DE( P 〈 0.05 ), LVEF obtained by sRT -3DE was lower than that by 2DE, there was significant difference between the two groups ( P 〈 0.05 ) , and there was good correlation between the two methods. Conclusion sRT - 3DE can get the real four - dimensional imaging ,it can measure the heart chamber volume and evaluate the heart function accurately, sRT - 3DE is superior to 2DE ultrasound in evaluation of left ventricle function in patients with enlarged heart chamber.
