新生儿肺炎克雷伯杆菌败血症21例临床分析 被引量:2

摘要 目的分析肺炎克雷伯杆菌败血症的临床特点、病原菌分布及药敏特点,为早期诊断和治疗提供临床依据。方法采取回顾性研究对该院新生儿重症监护室(NICU)确诊的21例肺炎克雷伯杆菌败血症患儿的临床特点及危险因素进行分析。结果 21例血培养均为肺炎克雷伯杆菌,其中19例(90.5%)为院内感染,以晚发感染为主。肺炎克雷伯杆菌17例(81.0%)为产β内酰胺酶(ESBLS)菌株,对碳青霉烯类、喹诺酮类100%敏感,哌拉西林/他唑巴坦敏感率76.47%,氨基糖苷类敏感率36.58%,头孢吡肟敏感率10.52%,对青霉素及2、3代头孢100%耐药。21例患儿均治愈出院。结论肺炎克雷伯杆菌是NICU中败血症的主要致病菌,多为院内感染,与早期使用3代头孢、早产儿、侵入性操作关系密切,碳青霉烯类、哌拉西林/他唑巴坦为治疗首选药物。
