288例子宫内膜异位症患者临床及预后分析

Clinical and Prognostic Analysis of 288 Patients with Endometriosis

目的探讨288例子宫内膜异位症患者的临床特点、治疗方法及预后。方法回顾性分析288例子宫内膜异位症患者的临床资料,随诊时间6～48个月。结果 288例子宫内膜异位症患者主要临床表现为慢性盆腔痛(58.3%),痛经(33.3%)、盆腔包块(27.8%);合并其他妇科疾病者占65.3%;AFS评分Ⅰ期18.1%,Ⅱ期18.1%,Ⅲ期30.5%,Ⅳ期33.3%;采用保留生育功能手术者54.1%,保留卵巢功能手术者29.2%,根治性手术者16.7%;术后药物治疗者58.3%,总复发率12.1%。结论子宫内膜异位症患者手术治疗后复发与其年龄、临床分期、临床病理类型、手术方式及术后是否用药有关。

Objective To investigate the clinical characteristics,treatment,and prognosis of 288 patients with endometriosis.Methods The clinical records of 288 patients with endometriosis were analyzed retrospectively.Results The most common symptoms were chronic pelvic pain（58.3%）,dysmenorrhea（33.3%）,and pelvic mass（27.8%）.Of the patients,65.3% had other gynecological diseases simultaneously.According to American Fertility Society score,18.1% of the patients were in stage Ⅰ,18.1% in stage Ⅱ,30.5% in stage Ⅲ,and 33.3% in stage Ⅳ.The proportions of patients undergoing conservative surgery,semi-radical mastectomy,radical mastectomy were 54.1%,29.2%,and 16.7%,respectively.After surgery,60.3% of the patients received drug therapy,and the overall recurrence rate was 12.1%.Conclusion The recurrence of endometriosis after surgery may relate to the patient＇s age,clinical stage,pathological type,surgical approaches,and postoperative medication.
