TGF-β_1、TβR-Ⅱ在鼻内翻性乳头状瘤中的表达及其临床意义

摘要 目的探讨转化生长因子β(1TGF-β1)及Ⅱ型受体(TβR-Ⅱ)在鼻内翻性乳头状瘤(NIP)中的表达及其在该病恶变中的临床意义。方法将72例NIP分为NIP良性病变组、NIP不典型增生组、NIP恶变组,应用免疫组织化学SABC法检测各组中TGF-β1和TβR-Ⅱ蛋白的表达,同时取10例鼻息肉做对照。结果 TGF-β1在鼻息肉组、NIP良性病变组、不典型增生组、恶变组的阳性表达率分别是30.00%,34.09%,68.75%,91.67%;TβR-Ⅱ在鼻息肉组、NIP良性病变组、不典型增生组、恶变组的阳性表达率分别是40.00%,40.91%,12.50%,0.00%。NIP恶变组中TGF-β1表达水平高于NIP良性病变组和鼻息肉组(P<0.05),而NIP恶变组中TβR-Ⅱ表达水平低于NIP良性病变组和鼻息肉组(P<0.05);NIP不典型增生组中TGF-β1表达水平与NIP恶变组比较无统计学差异(P>0.05),NIP良性病变组中TGF-β1的表达水平与鼻息肉组比较无统计学差异(P>0.05)。3组NIP中,TGF-β1和TβR-Ⅱ表达均呈负相关(rs=-0.47,P<0.01)。结论 TGF-β1、TβR-Ⅱ表达异常与NIP恶变密切相关,但与NIP的发生可能无关;对NIP伴不典型增生的病例进行TGF-β1和TβR-Ⅱ蛋白的联合检测,有助于预测NIP恶变。 Objective To detect the expression of TGF-β1 and TβR-Ⅱ in nasal inverted papilloma(NIP)and investigate their clinicopathological significances in occurrence and aggravation of this disease.Methods Specimens of NIP were obtained from 72 patients undergoing surgery.They were divided into three groups:NIP with benign form(44),NIP with epithelial dysplasia(16),and NIP with malignant transformation(12).Above groups were studied for TGF-β1 and TβR-Ⅱprotein expression using immunohistochemistry and compared with 10 nasal polyps(NP).Results The positive rates of TGF-β1 protein were 30% in NP,34.09% in NIP with benign form,68.75% in NIP with epithelial dysplasia and 91.67% in NIP with malignant transformation respectively.The expression of TGF-β1 in NIP with malignant transformation was significantly higher than that in NIP with benign form and in NP(P〈0.05),but no significant differences in NIP with epithelial dysplasia(P〉0.05).Meanwhile,the expression of TβR-Ⅱ were 40%,40.91%,12.5% and 0% in above four groups in turn.It showed there were significant differsnces between NIP with benign form and NIP with malignant transformation(P〈0.05),but no significant differences between NIP with malignant transformation and NIP with epithelial dysplasia(P〉0.05).Besides,the expression of TGF-β1 protein was negatively correlated to that of TβR-Ⅱprotein in the 72 cases of NIP tissue.Conclusion The abnormal expression of TGF-β1 and TβR-Ⅱ may be closely correlated with NIP of its malignant process,but have no relation with its occurrence.The combination analysis of TGF-β1 and TβR-Ⅱimmunohistochemical expression in NIP with epithelial dysplasia tissue may be useful for predicting malignant transformation.
