颈椎退变性滑脱的诊断与治疗 被引量:3

摘要 [目的]回顾性分析颈椎退变性滑脱的诊断标准与治疗方法的选择。[方法]一组退变性颈椎滑脱进行手术治疗的患者共52例,其中男40例,女12例,平均年龄58岁。术前均拍摄颈椎侧位和伸屈侧位X线平片,在标准侧位片,按下位椎体上终板水平测量滑脱节段椎体滑移距离。治疗方法选择滑脱和脊髓受压节段施行颈前路减压植骨内固定术,并采用JOA评分对术前及术后神经功能进行对照。[结果]所有病例颈椎滑脱测量结果:滑脱距离2.3~4.5mm,平均3.2mm;其中上位椎体向前滑脱22例,向后滑脱30例。32例颈椎的生理曲度消失。52例均行颈椎前路减压植骨融合内固定术,平均获得1.7年随访,所有病例均获得良好骨性融合,根据JOA评分标准,平均改善率为88.4%,优39例,良5例,中2例。[结论]在X线侧位片上相邻椎体位移大于2mm可诊断为颈椎滑脱;单纯颈痛患者的手术治疗必须谨慎;颈前路减压与植骨融合效果满意。
