骨炎性肌纤维母细胞瘤的影像表现

Imaging Appearance of Bone Inflammatory Myofibroblastic Tumor

目的:探讨骨炎性肌纤维母细胞瘤的影像学表现,以期提高对本病的认识。材料和方法:回顾性分析8例经病理证实的骨炎性肌纤维母细胞瘤。结果:发病部位为长骨干骺端3例(股骨2例,胫骨1例),上颌骨2例,下颌骨、肩胛骨及颧骨各1例。X线和(或)CT检查11例,均为不同程度骨质膨胀、溶骨性骨质破坏,内有条索状增粗骨小梁4例,软组织肿块5例,骨膜反应1例,肺内转移2例;MRI检查3例,表现为T1WI及T2WI均为混合信号,T1WI以等或低信号为主,T2WI以高信号为主,合并软组织肿块2例,增强扫描均为不均匀强化,2例液化坏死区无强化。结论:骨炎性肌纤维母细胞瘤具有恶性骨肿瘤的影像学特点,影像学检查在该病的临床诊断及鉴别诊断中有重要的价值。

Purpose To study the imaging appearances of bone inflammatory myofibroblastic tumor. Meterials and Methods The imaging appearances of bone inflammatory myofibroblastic tumor were reviewed in 8 patients, in wich the diagnoses were confirmed by histopathology. Result In 8 cases with bone inflammatory myofibroblastic tumor, tumors were located in metaphysis of long bone in 4 cases （included femur 2 cases and tibia 1 case） and maxillary 2 cases, mandible scapula and zygomatic 1 case respectively. There were 11 lesions by x-ray and（or） ct scans, all appeared osteolytic destruction with expansion,4 cases showed cord-like thickening trabecular,with soft- tissue mass in 5 cases,with lung metastasis in 2 cases and only 1 case with periosteal reaction. 3 cases presented mixed signal intensity with compar,intermediate signal intensity on T1 and T2-weighted images, hypointensity or intermediate signal intensity on T1WI, high signal intensity and with slightly hypointensity on T2WI,with soft-tissue mass in 2 cases. The tumors prented enhancement with low dense area inside the 2 lesions after injection of contrast media. Conclusion Bone inflammatory myofibroblastic tumor with the imaging features of malignant bone tumor, the imaging examination can provide helpful information for the clinical diagnosis and differential diagnosis of bone inflammatory myofibroblastic tumor.
