864例次耐亚胺培南铜绿假单胞菌医院感染特征分析

Nosocomial Infection of Imipenem-resistant Pseudomonas aeruginosa:Analysis of Characteristics of 864 Case-times

目的了解耐亚胺培南铜绿假单胞菌(IRPA)医院感染特征。方法对全国医院感染监控网医院报道的864例次IRPA医院感染进行统计分析。结果 IRPA感染者除损伤和烧伤组外,中位年龄均>60岁;感染前的平均住院天数(25.99±26.50)d,按基础疾病分类,以胰腺疾病者住院时间最长达(36.53±33.61)d,所有组相比,差异有统计学意义(χ2=43.38,P<0.01);按感染部位分,以泌尿道感染者住院时间最长达(35.82±39.64)d,各部位感染组相比,差异有统计学意义(χ2=23.51,P<0.05);IRPA医院感染者泌尿道及动静脉插管、呼吸机、免疫抑制剂、抗肿瘤化学治疗的使用率高于非IRPA医院感染者,差异有统计学意义。结论 IRPA医院感染者以老年患者为主,发生感染之前平均住院时间长,患者侵入性操作多。

OBJECTIVE To get the message of nosocomial infection of imipenem-resistant Pseudomonas aeruginosa（IRPA）.METHODS To analyze the characteristic of 864 case-times with nosocomial infection of IRPA which were reported by nosocomial infection surveillance system.RESULTS Patients with pulmonary disease infected IRPA were the oldest,and the average age was （71.91±13.35） years.The median age was more than 60 years except for the injury and burn patients.Before been infected the average hospitalization days were（25.99±26.50）d.According to the different underlying diseases to classify,patients with pancreatic disease had the longest hospital staying time （36.53±33.61）d.All of the groups compared with each other there were significant differences （χ2 =43.38,P0.01）.According to the different infection sites to classify,patients with urinary tract infection had the longest hospital staying time （35.82±39.64）d.All of the groups compared each other there were significant differences （χ2 =23.51,P0.01）.The usage rates of urinary tract and arteriovenous catheter,ventilator,immunosuppressant,tumor chemotherapy of patients with IRPA nosocomial infection were higher than patients without IRPA nosocomial infection,and there were significant differences.CONCLUSION Most of patients with IRPA nosocomial infection are the elderly,and there are longer average hospitalization days and more invasive operations before patients were infected.
