家庭无创通气并家庭氧疗治疗慢性阻塞性肺疾病稳定期并Ⅱ型呼吸衰竭疗效分析 被引量:4

摘要 目的研究慢性阻塞性肺疾病(COPD)稳定期并Ⅱ型呼吸衰竭患者使用家庭无创通气并家庭氧疗的疗效。方法将25例COPD稳定期并Ⅱ型呼吸衰竭患者按随机数字表法分为治疗组及对照组。治疗组12例使用双水平无创正压通气,每日治疗4—8h,治疗间期给予低流量吸氧,双水平无创正压通气加上低流量吸氧总时间15h以上。对照组13例给予长期家庭氧疗,每日低流量吸氧15h以上。结果治疗组治疗6个月后,PaCO2与对照组比较差异有统计学意义(P〈0.01),6min行走距离、FEV1%、FEV1/FVC、圣乔治呼吸问卷评分分别为(230.2±15.8)m、(42.2±9.6)%、38.0±3.4、(69.1±2.7)分,与对照组的(217.4±8.3)m、(38.1±8.7)%、35.3±2.4、(71.8±2.6)分比较差异有统计学意义(P〈0.05),pH值、PaO2与对照组比较差异无统计学意义(P〉0.05)。结论家庭无创通气并家庭氧疗是治疗COPD稳定期并Ⅱ型呼吸衰竭患者积极和有效的治疗措施,值得进一步研究。
