雷贝拉唑联合瑞巴派特治疗活动性胃溃疡36例

Effect of Rabeprazole Combined with Rebamipide in the Treatment of 36 Patients of Active Gastric Ulcer

目的评估国产雷贝拉唑联合瑞巴派特治疗幽门螺杆菌(Hp)感染活动期胃溃疡的临床症状、内镜下黏膜变化及组织学炎症的改善情况,随访患者6个月、1a溃疡复发率。方法采用随机平行对照临床研究,将72例Hp感染的活动期胃溃疡患者随机分为治疗组和对照组,各36例。治疗组:第1周,给予Hp三联方案之一(雷贝拉唑+阿莫西林+克拉霉素)联合使用瑞巴派特,瑞巴派特每次100mg,tid;第2～8周,雷贝拉唑每次10mg,qd,联合瑞巴派特,每次100mg,tid,po;对照组:第1周,给予Hp根除三联方案(奥美拉唑+阿莫西林+克拉霉素);第2～8周,予奥美拉唑每次20mg,qd,po。治疗8周末行胃镜检查及病理检查,随诊所有患者6个月、1a溃疡复发率。结果治疗后,两组临床症状均有显著改善,第8周末复查胃镜下溃疡愈合情况及黏膜炎症均明显改善;治疗组8周末胃镜下溃疡黏膜活动性炎症改善比对照组明显,随访1a的溃疡复发率较对照组低,差异有显著性。结论雷贝拉唑联合瑞巴派特治疗活动性胃溃疡能够更好缓解临床症状,促进炎症消退,提高溃疡愈合质量,减少溃疡复发率。

Objective To evaluate therapeutic effects of rabeprazole combined with rebamipide on active peptic ulcer of Helicobacter pylori infection,including clinical symptoms,endoscopic mucosal inflammatory changes,and the ulcer recurrence rate in six-month and 1-year follow up observation. Methods A randomized controlled clinical study in parallel was carried out. 72 cases of gastric ulcer with active Hp infection were randomly divided into treatment group and control group,36 cases in each. In the treatment group： Hp eradication triple plan combined with rebamipide was given at the first week,100 mg tid; at the following 2 to 8 week,rabeprazole 10 mg,qd,and rebamipide,100 mg tid. For the control group： Hp eradication triple plan was taken for the first week; and omeprazole 20 mg per day was administered in the following 2 to 8 weeks. At the end of 8 weeks, gastroscopy and pathological examination were performed,and the ulcer recurrence rate were observed in the follow-up six-month and 1-year for all patients. Results After treatment,clinical symptoms like mucosal ulcer healing and inflammation were significantly improved under gastroscopy and endoscopy,compared with the control; and 1-year follow up ulcer recurrence rate was lower than the control,the difference showed statistically significance. Conclusion Rabeprazole combined with rebamipide could alleviate ulcer symptoms,subside inflammation,improve the quality of ulcer healing and reduce ulcer recurrence rate.
