摘要
目的应用脑氧饱和度监测手段探讨颈动脉体瘤手术围术期脑氧供需平衡的管理与维护特点。方法通过对20例颈动脉体瘤患者围术期脑氧饱和度(rSO2)、动脉压和颈内动脉反流压等监测,维护患者术中脑氧供需平衡。结果颈动脉体瘤患者阻断患侧颈内动脉期间rSO2与阻断前、后比较均有显著性差异(P<0.05),术侧rSO2在阻断中较非术侧显著下降(P<0.05);术侧阻断后rSO2在开放后的一段时间内未能恢复到阻断前水平(P<0.05),3例患者因术中反流压过低行动脉旁路术。结论术中rSO2监测对患者围术期脑氧供需平衡的管理具有重要指导意义。
Objective To investigate the management of cerebral oxygen balance during carotid body tumor surgery by monitoring near-infrared spectrometer (rSO2).Methods Twenty patients were enrolled,the rSO2,artery blood pressure and carotid stump pressure were continuously monitored and recorded during the perioperative period.Results There was significant difference in rSO2 between the two sides; The rSO2 of patients during carotid clamping was different from before and after cross-clamping carotid.Three patients underwent carotid bypass catheterization because the carotid reverse blood pressure of internal carotid artery was too low.Conclusions The rSO2 monitoring during carotid body tumor surgery is very useful for cerebral oxygen balance monitoring for carotid tumor surgery.
