小儿过敏性疾病的变应原诊断

Allergen diagnosis in children with allergic diseases

目的制定过敏性疾病患儿常见的变应原组合,检查其灵敏度和特异度。方法选择124例过敏性疾病患儿,通过评估其过敏史,应用21种常见变应原进行皮肤点刺试验,并抽取5ml静脉血行血清学IgE检查。结果根据变应原皮肤试验阳性率制定的变应原组合为粉尘螨、屋尘螨、德国小蠊、艾蒿、大豚草、动物毛以及霉菌组合;该变应原组合其阳性率为99%,特异性为79%,阳性预测值和阴性预测值分别为96%和94%;准确性为96%。结论本研究所制定的变应原组合具有良好的诊断学指标,符合卫生经济学要求。

Objectives To investigate the common allergen in children with allergic diseases, and to analyze its sensitivity and specificity.Methods A study on clinical data was carried out among 124 cases of children with allergic diseases during March 2008 to March 2009.Skin prick tests of 21 common allergens and serum IgE level was tested.Results The allergen-grouping based on positive rate of skin prick tests including Dermatoph.farinae, Dermatoph.Pteronyssinus, Blatella Germanica, mugwort, ragweed, animal dander and mycetes.The positive rate of this allergen-grouping was 0.99, and specificity was 0.79.The positive and negative predictive value was 0.96 and 0.94 respectively, and the accuracy rate was 0.96.Conclusions The allergen-grouping formulated by this study has good diagnostics parameters, and it is cost-effective.
