先天性胆总管囊肿切除间置空肠代胆道术后常见并发症及处理

The common post-operative complications and management of congenital choledochal cyst excision and jejunal interposition with spur antireflux valve

通过对１８７例行胆总管囊肿切除、间置空肠代胆道矩形瓣返流术后患儿的随访，提出术后应激性溃疡、小肠套叠、胆瘘、十二指肠肠瘘及急性肝功能衰竭力近期并发症，而粘连性肠梗阻、反复胆道感染及慢性胰腺炎等则为远期并发症。本文指出规范手术操作、减少手术打击、保护肝脏、加强术前、术中、术后监护可降低并发症的发生。

187 cases performed with congenital choledochal cyst excision and jeiunalinterposition with apur antireflux valve were followed up. The short term complications are stressulcer, enteric intussusceptionm, binary fistula, duodenal fistula and acute hepatic failure, whilethe long term ones are adhesive lieus, repeated infection of biliary tract and chronic pancreatitis. Itis suggested that to perform operation properly, to minimize operative trauma, to protect the liverand to straighten monitor prior, during and after the operation can reduce the occurrence of thecomplications.
