硬膜外阻滞复合全麻在腹腔镜胆囊切除术中的应用研究

Combined Epidural-General Anesthesia Used for Laparoscopic Cholecystectomy

目的探讨硬膜外阻滞复合全麻用于腹腔镜胆囊切除手术(LC)的可行性、安全性。方法随机将LC60例患者分为对照组(静脉全麻)与观察组(静脉全麻复合硬膜外麻醉),每组30例。观察术中血压(BP)、心率(HR)、吸入全麻药浓度及麻醉苏醒时间的变化。结果两组患者气腹前后及拔管5min、10minHR、SBP、DBP变化比较差异有统计学意义(P<0.05),术毕患者苏醒时间对照组为(21.8±5.5)min,观察组为(11.9±4.4)mim,两组比较差异有统计学意义(P<0.05)。结论 LC全麻复合硬膜外麻醉对循环和呼吸的干扰小,清醒质量高且经济安全。

Objective To study the feasibility and safety of the epidural block combining with the venous general anesthesia（ EBCVGA）applied on patients who are conducted the laparoscopic cholecystectomy （LC）. Methods Totally 60 cases underwent LC were divided into two groups randomly, there were 30 cases every, group. The blood pressure（BP） ,heart rate（ HR）and end -tidal concentration of enflurane were continuously monitored intraoperatively. The time of anesthetic recovery was evaluated in time of patients completed the control group （21.8±5.5） rain,observation of group （11.9±4. 4） mira, difference between the two groups was statistically significant （ P 〈 0. 05 ）. Conclusion The intrave- nous anesthesia combined with epidural anesthesia in laparoscopic cholecystectomy are safe and economical with quickly wake ,little disturb to respiratory apparatus an d reeirculating system.
