布-加综合征分布与饮用水碘含量关系研究

Relationship between Distribution of Budd-Chiari Syndrome and Iodine Content of Drinking Water

目的探讨外周环境饮用水中碘含量与布-加综合征发病的关系。方法选择1995—2004年菏泽市立医院等5家医院收治的居住地为山东菏泽且可以查到地址的342例布-加综合征患者为研究对象,对其性别、年龄、饮用水中碘含量进行描述性分析;以2003年山东省菏泽地区各乡镇的水碘中位数为依据,建立菏泽地区布-加综合征患者空间分布图,并对水碘与布-加综合征关系进行空间统计分析;以2000年第五次人口普查数据为依据,计算菏泽各县布-加综合征发病率,并对各县布-加综合征发病率与水碘值中位数进行线性相关分析。结果在342例布-加综合征患者中,女性126例,占36.84%;男性216例,占63.16%,男女性别比为1.71:1。患者年龄为(40.30±12.97)岁,300例患者分布于25～64岁年龄段。菏泽地区布-加综合征患者所在乡镇的水碘含量范围为12.62～681.21μg/L,中位数为262.05μg/L。89.18%患者所在乡镇的水碘值>150μg/L,水碘浓度与布-加综合征发病率呈正相关(r=0.846,P=0.008)。结论布-加综合征的发生可能与饮用水中碘含量过高有关,高碘在布-加综合征发病过程中的作用还有待进一步的研究。

Objective To explore the relationship between iodine content of drinking water and Budd-Chiari syndrome （B-CS）. Methods 342 cases of B-CS, local residents of Heze, Shandong province,were selected in five hospitals in 1995-2004. The gender, age, iodine content of drinking water and distribution of patients in Heze were analyzed, and the relationship between water iodine and B-CS was studied by the statistical analysis, based on the median iodine of different townships of Heze, Shandong province, in 2003. The incidence of county level was determined according to the fifth census data of different counties in 2000. Results In 342 cases of Budd-Chiari syndrome, females accounted for 36.84%（126）, males accounted for 63.16%（216）, the ratio of male to female was 1.71：1. Age of patients was （40.30±12.97） years, from 25 to 64. The range of water iodine concentration was 12.62-681.21μg/L in townships of Heze area where B-CS eases existed, median was 262.05μg/L. 89.18% water iodine concentration in where B-CS cases existed was higher than 150 μg/L, water iodine concentration was positively correlated with the incidence of B-CS （r=0.846, P=0.008）. Conclusion B-CS may be related with the high iodine in drinking water in living environment, and more attention should be paid to the research on the role of high iodine in the pathogenesis of B-CS.
