摘要
目的:探讨食管鳞状细胞癌中基质金属蛋白酶-1(MMP-1)和血管内皮生长因子-C(VEGF-C)的表达及其与食管鳞状细胞癌临床病理参数的关系。方法:应用免疫组织化学S-P法检测90例食管鳞状细胞癌组织、16例重度不典型增生/原位癌、28例癌旁正常上皮组织中MMP-1及VEGF-C的表达情况。结果:MMP-1在食管鳞状细胞癌、不典型增生/原位癌及癌旁正常组织中的阳性率分别为81.11%、81.25%及17.86%(P〈0.01),其表达与食管鳞状细胞癌浸润深度、淋巴结转移以及临床分期均有一定关系,而与分化程度无明显关系(P〉0.05)。VEGF-C在食管鳞状细胞癌、不典型增生/原位癌及癌旁正常组织中的阳性率分别为83.33%、75.00%及25.00%(P〈0.01),其表达与食管鳞状细胞癌淋巴结转移及临床分期均有一定关系(P〈0.01和P〈0.05),而与分化程度及浸润深度均无明显关系(P〉0.05)。两者呈正相关关系(P〈0.01)。结论:MMP-1、VEGF-C两者协同促进了食管鳞状细胞癌的浸润与淋巴结转移,可作为判断食管鳞状细胞癌生物学行为的重要参考指标。
Objective:To investigate the expression of matrix metalloproteinase-1(MMP-1) and vascular endothelial growth factor-C(VEGF-C) in human esophageal squamous cell carcinoma(ESCC),and their relationship with the clinicopathological parameters.Methods:The expressions of MMP-1 and VEGF-C were determined immunohistochemically in 90 cases of human esophageal squamous cell carcinoma,16 cases of severe dysplasia/carcinoma in situ and 28 cases of adjacent normal tissues.Results:The positive expression rates of MMP-1 in ESCC,severe dysplasia/carcinoma in situ and adjacent normal tissues were 81.11%,81.25% and 17.86%,respectively.The expression of MMP-1 was associated with the depth of invasion,lymph node metastasis and clinical staging in ESCC,but not with differentiation(P〈0.05).The positive expression rates of VEGF-C in ESCC,severe dysplasia/carcinoma in situ and adjacent normal tissues were 83.33%,75.00% and 25.00%,respectively(P〈0.01) ;the expression of VEGF-C was associated with the lymph node metastasis and clinical staging in ESCC(P〈0.01 and P〈0.05),but not with the differentiation or depth of invasion(P〉0.05).And there was a positive correlation between MMP-1 and VEGF-C(P〈0.01).Conclusions:MMP-1 and VEGF-C play a crucial role in invasion and lymphatic metastasis.They are important predictors of biological behavior in ESCC.
