小儿右心室异常肌束的诊断和治疗

THE DIAGNOSIS AND TREATMENT OF ANOMALOUS MUSCLE BUNDLE OF RIGHT VENTRICLE IN CHILDREN

本文报道经手术证实的10例右心室异常肌束,其中9例伴有室间隔缺损。临床表现与室间隔缺损、肺动脉瓣狭窄、法乐四联症相似,术前诊断困难。右心导管结合右心室造影有助诊断。彩色多普勒超声心动图检查是一种简单、安全、正确的诊断方法。提出术中诊断和手术要点。本组无死亡或并发症。

Anomalous muscle bundle of right venbriole(AMB) is a congenital disease which may cause intracardiac obstruction. It is often associated with ventricular septal defects thus making clinical detection difficult. Ten oases of AMB verified through operation are reported. Nine of them had associated ventricular septal defect and one had an isolated AMB lesion. The differentiation of these patients from those with ventricular septal defect alone, pulmonary stenosis, or tetralogy of Pallet was difficult because of similarities in clinical manifestations. Eight heart oatheterization combined with selective angiocardiography were helpful in diagnosis. The color Doppler eohocardiography proved to be an excellent diagnostic method. It is simple, safe, accurate and noninvasive. There were no postoperative complications or deaths in this series. The cruxes of operative diagnosis and surgical technique are proposed.
