急性乳酸酸中毒对实验犬心室收缩功能影响的研究

Effect of lactic acidosis on ventricular systolic function in dogs

目的：观察实验性乳酸酸中毒对犬心室收缩功能的影响。方法：应用超声心动图和血流动力学监测技术，观察９只经静脉麻醉、气管插管、连续静脉滴注０．５ｍｏｌ／Ｌ的ＤＬ乳酸液后，健康杂种犬心室收缩功能变化；每３０分钟采血液、心室、主动脉压力及超声心动图数据１次。结果：主动脉瓣血流平均加速度、左室射血分数、左室周边缩短速率、肺动脉瓣血流平均加速度、右室面积变化率和右室射血分数均降低（Ｐ均＜０．０５），心室收缩末径较舒张末径增大更明显（Ｐ均＜０．０５）。结论：乳酸酸中毒降低心室收缩功能，尤其降低心肌收缩力。

Objective :To assess ventricular systolic function in dogs with lactic acidosis. Methods :Using two dimensinal and pulsed Doppler echocardiography and hemodynamics monitoring techniques,ventricular systolic function in dogs were observed.9 mongrel dogs (1898241)kg were anesthetized with pentothal sodium (thiopental sodium 30 mg/kg) and intubated.Then 05 mol/L DL lactic acid was infused continuously to animals at 10 mgkg-1min-1.Data were collected every 30 minutes during acid infusion until death. Results :During progressive acid infusion,the acceleration of the aortic ejection,the ejection fraction of the left ventricle,the velocity of circumferential fiber shortening,the acceleration of the pulmonary ejection,fractional shortening of the area of the right ventricle,and the ejection fraction of the right ventricle were all decreased (all P<005),the enlargement of ventricle endsystole dimension was more obvious than that of ventricle end diastole dimension (all P<005). Conclusions :Lactic acidosis can lead to a decrease in ventricular systolic function,especially there was a depression of myocardial contractility.
