我院甲状腺/乳腺/疝手术围术期应用抗菌药物分析

Analysis of Perioperative Prophylactic Use of Antibacterials in Patients Undergoing Thyroid Gland/Mammary Gland or Hernia Surgery in Our Hospital

目的:了解我院Ⅰ类切口手术患者围术期预防性应用抗菌药物情况。方法:抽取我院2009年6-10月甲状腺/乳腺/疝手术病例360份,调查围术期抗菌药物的应用情况。结果:我院甲状腺/乳腺/疝手术抗菌药物应用率为98.05%。应用较多的药物为五水头孢唑林;术前0.5～2h应用抗菌药物占84.44%;术前>2h及术后开始应用抗菌药物分别占6.39%和7.22%;抗菌药物平均应用时间分有无危险因素分别为5.73d、2.88d,最长达10d。结论:Ⅰ类切口手术预防用药率高;医生选择药物抗菌谱合理,但费用过高;应用时机基本合理,但疗程过长。

OBJECTIVE：To investigate the perioperative prophylactic use of antibacterials in patients with type I incision surgery in our hospital.METHODS：The use of antibacterial drugs in 360 patients undergoing thyroid gland/mammary gland/hernia surgery in our hospital between June 2009 and October 2009 were investigated.RESULTS：98.05% of the total 360 cases received antibacterials.The utilization ratio of cefazolin sodium pentahydrate was the highest.84.44% received antibiotics 0.5～2 hrs before operation,6.39% 2 hrs before operation and 7.22% received antibiotics after operation.The average days of receiving antibiotics were 5.73 days in the presence of risk factors and 2.88 days in the absence of risk factors,with a maximum of 10 days.CONCLUSION：The perioperative utilization rate of antibacterials in type I incision surgery in our hospital was on the high side.Although the choice of antibacterial spectrum was rational,the costs of the antibacterials were high and the course of treatment is excessively long.
