2001年至2007年我院门诊平喘药物用药情况调查

Application of antiasthmatic drugs in outpatients of our hospital from 2001 to 2007

目的观察门诊平喘药物用药的变化趋势及与哮喘防治指南的关系。方法统计2001年至2007年我院每年门诊总例次和呼吸科门诊例次,收集平喘药物口服肾上腺素β_2受体(β_2受体)激动药、口服短效茶碱、口服缓释茶碱、口服白三烯受体拮抗药、吸入短效β_2受体激动药、吸入长效β_2受体激动药及吸入激素的全年用药量。以药物限定日剂量(DDD)及吸入激素的等效剂量为依据对用药量换算,计算出药物的用药频度(DDDs)及吸入激素等效剂量数。以2001年的DDDs(或吸入激素等效剂量数)为基数"1",计算出其余各年与2001年的DDDs(或吸入激素等效剂量数)的比值,即相对值。再将各相对值取常用对数(1g相对值)。结果 2001年至2007年我院呼吸科门诊例次和全院门诊总例次均有上升趋势。平喘药物使用中口服和吸入短效β_2受体激动药用量无增加,口服平喘药中白三烯受体拮抗药的用量明显增多。吸入糖皮质激素及吸入长效β_2受体激动药-糖皮质激素复合制剂用量增加,吸入制剂中,干粉吸入剂明显增加。结论我院门诊哮喘用药根据哮喘防治指南的修改而发生相应的变化。

AIM To investigate the application of antiasthmatic drugs in the outpatients with asthma of our hospital from 2001 to 2007 and its relationshop to guidelines for asthma management and prevention. METHODS Both the total outpatients and the respiratory outpatients of our hospital were collected from 2001 to 2007. The annual quantities of oral β2 adrenergic receptor （β2 receptor） agonist, oral short-acting theophylline, oral sustained-release theophylline, oral leukotriene receptor antagonist, inhaled short-acting β2 agonist, inhaled long-acting β2 agonist, inhaled corticosteroid and the complex preparation of long-acting β2-agonist and inhaled corticosteroid were surveyed. The DDDs and the equivalent doses of inhaled corticosteroids were calculated. RESULTS The total outpatients and the respiratory outpatients of our hospital from 2001 to 2007 showed a rising tendency. The use of oral and inhaled short-acting β2 agonists didn＇t increase in accordance with the increase of the outpatients. The use of leukotriene receptor antagonists increased significantly. The use of inhaled glucocorticoid and the complex preparation of long-acting β2 agonists and glucocorticoids increased. Among the inhalants, the use of dry powder inhalant （DPI） increased obviously. CONCLUSION Changes in the use of antiasthmatic medication for the outpatients with asthma in our hospital is consistent with the revision of the prevention and treatment guidelines of asthma.
