不同尿液引流方式对中重度尿道下裂患儿的疗效比较

Comparative Study on the Clinical Efficacy of Different Urinary Drainage in Children with Moderate and Severe Hypospadias

【目的】探讨不同尿液引流方式对中重度尿道下裂患儿的疗效。【方法】将本院2009年4月至2010年3月收治的中重度尿道下裂患儿67例随机分为两组。观察组：35例，尿道下裂术后经尿道支架管置入引流管至膀胱引流；对照组132例，尿道下裂术后膀胱造瘘引流。对两组临床效果进行比较观察。【结果】观察组术后一期治愈率为82．86％，略高于对照组的81．25％，但两组比较，差异无显著性（x^2=0．0294，P〉0．05）；观察组并发症发生率为2．86％，明显低于对照组的18．75％；两组比较，差异均有显著性（x^2=4．5128，P〈0．05）。【结论】尿道下裂术后经尿道支架管置入引流管至膀胱的引流方式对中重度尿道下裂患儿的疗效佳，且并发症较少，安全可靠，值得临床关注。

[Objective] To explore the clinical efficacy of different urinary drainage in children with moderate and severe hypospadias. [Methods]From April 2009 to March 2010 in our hospital, 67 patients of moderate and severe hypospadias were randomly divided into two groups. The observation group（ n = 35） underwent the operation of hypospadias followed by transurethral stent implantation on the drainage tube to the bladder drainage. The control group（ n = 32） underwent the operation of hypopadias followed by fistula bladder drainage. The clinical effect between two groups was compared. [Results] The cure rate of the observation group after operation was 82.86%, which was slightly higher than that of the control group（81.25 %）, but the difference had no statistical significance （3（ =0. 0294, P〉0.05）. The incidence of the complications in the observation group was 2.86%, which was significantly lower than that in the control group（18.75 %） , and the difference had statistical significance （X^2=4. 5128, P〈0.05）. [Conclusion] The operation of hypospadias followed by transurethral stent implantation on the drainage tube to the bladder drainage for the treatment of in children with moderate and severe hypospadias has good efficacy and less complications, and is safe and reliable, so it is worthy of clinical attention.
