摘要
目的研究血清尿酸(SUA)水平与高血压患者血管内皮功能(FMD)之间的相关性。方法门诊及住院的患者中随机选取高血压组患者(48例),体检的健康者为正常对照组(39例)作为研究对象。以血流介导的内皮依赖性血管舒张功能(FMD)和硝酸甘油引起的非内皮依赖性舒张功能(NMD)作为内皮功能的检测指标。同时根据SUA水平将高血压组分为尿酸正常组和高尿酸血症组。结果高血压组与正常对照组SUA分别为(363.69±90.27)μmol/L和(303.32±99.27)μmol/L,组间差异具有显著性,P<0.05。尿酸正常组与高尿酸血症组FMD分别为(10.27±4.79)%及(6.17±2.37)%,组间差异具有显著性,P<0.05。尿酸正常组中高血压1、2、3级FMD分别为12.84%、10.89%及6.29%(高血压1、3级,2、3级组间差别具有显著性,P<0.05);高尿酸血症组按高血压分级FMD分别为7.04%、6.39%及5.41%(各组间差别均无显著性,P>0.05)。随着高血压分级的增加FMD呈降低趋势;同级高血压患者的FMD随SUA的升高而降低。SUA与FMD呈负相关(r=-0.325),P<0.05。结论高血压患者SUA高于健康者;SUA水平与血管内皮功能受损相关;随SUA水平的升高FMD呈降低趋势。
Objective Through the detection of serum uric acid (SUA) and flow-mediated endothelium-dependent vasodilatation (FMD) for Hypertensive patients, we warlt to study the correlation between them and significance. Methods Select 48 hypertensive patients in Cardiovascttlar department of CJUH of Jilin University as hypertension group, 39 people that took health examination as the control group FMD andNMD were used as the detecting index of endothelial function. In addition, we divided hypertension group into nomlal UA group and hyperuricemia group. The focal point is studying the correlation between SUA and FMD in hypertdnsive patients. Results SUA of hypertension group and the control group is respectively (363.69± 90.27) μmol/L and (303.32± 99.27) μmol/ L, between the two groups with significant difference, p = 0. 004. Braehial artery diameter change percent of normal SUA group and hyperuricemia group is respectively ( 10.27 ±4.79) % and (6.17 ± 2.37) %, between the two groups with significant difference, P 〈 0. 05. NMD of the two group is respectively ( 14.19 ± 6.45 ) % and ( 10.75 ± 3.52) %, there' s no significant difference between them, P〉0.05. In patients who is high blood pressure level 1,2,3, the mean of FMD of normal uric acid group espectively is 12.84 %, 10.89 % and 6.29 % ; the mean of FMD of hyperuricemia group respectively is 7.04 %, 6.39 % and 5.41%. FMD descending with the increasing of the ranking of the high blood pressure; FMD of the same ranking blood pressure descending with the in- creasing of SUA. SUA is negative correlation with FMD( r = - 0.325), P 〈 0.05. Conclusion Level of SUA of hypertension group is hizher than the control group. SUA correlates with injury of FMD. FMD descending with the increasing of SUA.
