非体外循环下冠脉搭桥术483例临床分析

Clinical analysis of 483 off-pump coronary artery bypass grafting

目的总结非体外循环下冠脉搭桥术的临床经验,评价其疗效及安全性。方法收集2001年1月-2008年12月期间在我院心脏外科就诊的483例患者资料进行回顾性分析。患者均采用静吸复合麻醉进行搭桥术。共搭桥960支,平均每例2.3支(1～4支)。术后常规应用肝素和硝酸甘油,β-受体阻滞剂及血管活性药物。结果手术全部成功,术后死亡30例,死亡率6.21%。随访3～8个月均无活动后心前区疼痛症状,心功能及LVEF均有不同程度提高,生活质量提高。结论随着相关理论和手术操作技术的不断改善,非体外循环下冠脉搭桥术将成为一种较完善的冠心病治疗手段。

Objective To sum up the clinical experience of off-pump CABG and evaluate the effects and security of surgery.Methods 483 patients who have underwent off-pump CABG were collected from2001.1-2009.12,a total 960 grafts were performed and average of 2.3 grafts（1-4）was fixed up per patient.conventional heparin,nitroglycerin,β-receptor blockers,and vasoactive drugs were used postoperation.Results There have no postoperation death,all patients were followed up 3-8 months and angina were no found,cardiac function were 1-2 class,and the patients＇quality of life were improved.Conclusion With the improvement of theory and operative techniques,off-pump CABG will be a better treatment of coronary heart disease.
