摘要
目的研究兔子颈总动脉纵向拉伸以及吻合情况,为临床修复颈总动脉长短缺损提供实验依据。方法新西兰大白兔6只,均游离出长60mm颈总动脉,分3组(各2只)分别切除18、21和24mm,纵向拉伸后进行端端吻合,相当于23.8%、28.2%及33.3%的纵向伸长率。15d后对动脉血管进行病理学检查。结果切除18和21mm组颈总动脉吻合成功,切除21mm组吻合难度较大,切除24mm组吻合失败。吻合成功血管术后15d病理检查显示,动脉的吻合口平滑,血管内径无狭窄,血管存在水肿和纤维增生;血管内膜处于极度的拉伸状态。结论颈总动脉可被拉伸延长后进行端端吻合,伸长率可达28.2%。长段游离后拉伸动脉是有效的修复创伤性动脉长段缺损的方法之一。
Objective To investigate the consequences of the anastomosis on rabbits' common carotid artery which were cut and stretched to certain length to offer dependable foundation for using anastomosis in clinical application.Methods 6 rabbits were divided into 3 groups.60 mm carotid artery of a rabbit was separated and 18 mm,21 mm and 24 mm were cut off respectively for carrying out anastomosis by stretching,which count for 23.8%,28.2%,33.3% of the longitudinal elongation rate.Blood vessels were examined and the pathological examinations were done at 15 days after the anastomosis.Results The anastomosis of carotid artery with 18 mm and 21 mm cutting out off was successful with elongation rate of 23.8% and 28.2% respectively.The blood circulation of common carotid artery with 24 mm cut off was not fluent after anastomosis.15 days after the anastomosis.Pathological examinations showed that the successful anastomosis position of blood vessel was smooth and inner diameter was not narrower than that of other positions,but edema and fibrogenesis was found.Intima of blood vessel was extremely tense and intima cells were stretched in flat shapes.Conclusion Vascular experimental studies prove that the rabbit's common carotid artery can be stretched and end-to-end anastomosis can be done.Under the condition of 28.2% elongation the tense of vessel is not too much and the artery anastomosis can be performed successfully.The method of separating and stretching arteries for anastomosis is one of the most effective methods to repair traumatic long-segment defects of arteries.
