颅内动脉瘤栓塞术中脑局部血氧饱和度复合双频指数变化及意义 被引量:1

摘要 目的探讨脑局部血氧饱和度(rSO2)和脑电双频指数(BIS)判断颅内动脉瘤栓塞术中并发症的价值。方法 216例行颅内动脉瘤栓塞术患者,术中监测BIS及rSO2,麻醉维持平稳过程中出现异常(rSO2变化超过5%、BIS突然>70或<30)者立即行血管造影,并根据造影结果予相应处理,记录并发症发生前、发生时和处理后5min时的BIS值以及rSO2值及术后第2天神经功能障碍发生情况。结果术中38例出现BIS及rSO2异常,造影证实动脉瘤破裂6例,血管痉挛20例,血管栓塞12例。动脉瘤破裂者rSO2、BIS值变化幅度最大,栓塞者次之。治疗后5min动脉瘤破裂者rSO2、BIS值无明显变化,而血管栓塞和痉挛者rSO2、BIS值升高;术后第2天,血管破裂者3例昏迷,2例并发偏瘫;血管栓塞者1例出现偏瘫;血管痉挛者术后无1例出现神经功能障碍。结论 rSO2复合BIS可及时发现颅内动脉瘤栓塞术中动脉瘤破裂、血管栓塞或痉挛等并发症,并可预测神经功能障碍的发生。
