继发型肺结核患者血清瘦素水平与其营养状况相关性研究

Study on the correlation between leptin levels and nutritional state of the initially treat pulmonary tuberculosis patients

目的通过对继发型肺结核初治患者治疗前后血清瘦素水平、营养状况指标的测定,了解初治肺结核患者血清瘦素水平与其营养状况,初步探讨血清瘦素是否参与肺结核营养不良的发生。方法采用酶联免疫吸附(ELISA)双抗夹心法测定我院住院35例成人继发型肺结核初治患者抗结核治疗2个月前后的血清瘦素水平,同时检测入选者营养状况的变化。并以同期15例我院健康体检者为对照组。结果①肺结核组在治疗前后血清瘦素水平以及营养状况均低于健康对照组,P<0.05,在强化抗结核治疗2个月后,血清瘦素水平有所升高,P<0.05,各项营养指标中以清蛋白(ALB)、前清蛋白(PA)升高明显,P<0.01,体重指数(BMI)、腰/臀(WHR)有所改善,但P>0.05;②肺结核组治疗前后血清瘦素水平与ALB、PA、BMI均表现为正相关,P<0.05,健康对照组血清瘦素水平与ALB表现为负相关,P<0.05,与PA、BMI无相关性,P>0.05,血清瘦素水平与WHR无论在健康对照组,还是在肺结核组治疗前后均无相关性,P>0.05。结论血清瘦素可能参与了肺结核营养不良的发生。

Objective To study the relationship between the Leptin levels in blood serum and nutritional state of the body by detected those index of the patients with initially treated pulmonary tuberculosis patient′s.To study and research whether Leptin takes part in the pathology process of the pulmonary tuberculosis and the occurrence of malnutrition. Methods Using enzyme linked immunosorbent assay（ELISA） to detect the blood serum leptin levels and nutritional state of the 35 cases pulmonary tuberculosis patients before and after 2 month treated by antituberculosis drugs.15 health examination people were selected as normal control group.Results ①The levels of leptin and nutritional status of pulmonary tuberculosis group were significantly lower than those of healthy control group,P〈0.05.After treat by antituberculosis drugs the blood serum leptin levels of patients with pulmonary tuberculosis is heightened,P〈0.05.ALB,PA heighten obviously,P〈0.01,BMI,WHR is improved but have not statistically significant,P〈0.05,②The levels of leptin have significantly positive correlation with ALB、PA.BMI between prior and post treatment by antituberculosis drugs,P〈0.05.The levels of leptin have negative correlation with ALB,have not correlation with PA.BMI,P〈0.05.The levels of leptin have not correlation with WHR at control group or prior treatment and post-treatment group,P〈0.05.Conclusion Leptin may takes part in the pathology process of the pulmonary tuberculosis and the occurrence of malnutrition.
