CT在胃肠道间质瘤诊断中的价值

CT diagnostic value of gastrointestinal stromal tumors

目的分析胃肠道间质瘤(GIST)的CT表现特点,探讨CT对该肿瘤的诊断价值。方法回顾性分析35例经手术病理证实的GIST患者的CT资料。结果肿瘤发生于胃15例、小肠8例、结直肠5例、食管2例、腹膜后3例和肠系膜2例。35例GIST中良性15例,肿瘤直径≤5.0 cm,边界清楚、密度均匀。恶性20例,肿块直径>6 cm,肿瘤边缘有分叶,肿块内有坏死,周围组织侵犯或远处转移。本组CT定位准确率为100%(35/35)。CT判断良恶性准确度为88.6%(31/35)。结论 CT对GIST的诊断和鉴别诊断具有一定的价值。

Objective To analyze CT features of gastrointestinal stromal stumors（GIST）and to evaluate its value in diagnosis of the disease.Methods 35 patients with CT examination and histological diagnosis of GIST were restrospectively reviewed.Results The tumor originated from the stomach（n=15）,small intestine（n=8）,colorectum（n=5）,esophagus（n=2）,restroperitoneum（n=3） and mesentery（n=2）,of 35 GIST,15 were benign and 20 were malignant.In benign GIST,the diameter of the tumor was less than 5cm,with clear margin and homogeneous density.In malignant GIST,the diameter of the tumor was more than 6cm,the tumors were lobular in shape,with necrosis within the tumor,invasion of adjacent tissues and metastasis,The positive predictive value of CT for ocation of GIST was 100%（35/35）,the positive predictive value of CT for differentiation of benign and malignant GIST was 88.6 %（31/35）.Conclusion CT scan is of certain value in diagnosis and differentiating diagnosis of gastroroin testinalstromal tumors.
