Orem自理理论在腹腔镜胆囊切除术后护理中的应用

Utilization of Orem′s theory of self-care in patient with undergoing laparoscopic cholecystectomy

目的探讨Orem自理理论在腹腔镜胆囊切除术后护理中的应用效果。方法将1200例腹腔镜胆囊切除术患者随机分为对照组和实验组,对照组按术后常规进行护理,实验组在此基础上应用Orem完全补偿系统,部分补偿系统和支持教育系统进行护理干预,观察两组术后下床活动时间、肛门排气时间、肠蠕动恢复时间、患者对相关知识的掌握情况、患者满意度。结果与对照组比较,实验组术后下床活动时间明显提前(P<0.01),术后肠蠕动恢复时间明显缩短(P<0.01),患者对疾病的认知度提高,护理满意度提高。结论将Orem自理理论用于腹腔镜胆囊切除术后护理中,能调动和激发患者主观能动性,患者对疾病知识详细了解,提高自我护理能力,促进患者早日康复,增进了护患关系。

Objective To evaluate the efficacy of Orem′s theory of self-care in patient with undergoing laparoscopic cholecystectomy.Methods 1200 patients with undergoing of laparoscopic cholecystectomy were randomly divided into control group and test group.The control group were treated with routine nursing,and than the test group were treated with Orem complete back-off system、partial back-off system,supporting educational system and nursing intervene.It was recorded to the time for out-of-bed activity,gastrointestinal peristalsis and passage of gas by anus.The degree of satisfaction of nursing and degree of patient＇s cognition were investigated.Results Comparing with the control group,the test group was shortening the time of out-of-bed activity and gastroimestinal peristalsis,raising the degree of cognition,heightening degree of satisfaction,there were significant difference between two group（P〈0.01）.Conclusion Orem′s theory of self-care in patient with undergoing of laparoscopic cholecystectomy could mobilize patient′s subjective initiative,enhance the patient′s cognition,raise self-nursing capability,promote patients＇ recovery and upstep nurse-patient relationship.
