心血管疾病患者血清高敏C反应蛋白与心肺适能的相关性研究 被引量:3

摘要 目的探讨心血管疾病患者炎症因子高敏C反应蛋白(hs-CRP)与心肺适能(CRF)的相关性。方法选择行经皮冠状动脉介入治疗患者16例,稳定型心绞痛21例,原发性高血压23例。采用症状限制性标准Bruce分级平板运动方案,同时测定气体代谢指标摄氧量(VO2)、二氧化碳排出量(VCO2)、VO2峰值(VO2peak)、每分钟通气量(VE)和无氧阈值(VAT)。应用全自动免疫散射比浊法检测血清hs-CRP。结果血清10g(hs-CRP)与VOapeak(r=-0.697,P〈0.001)、VE峰值(VEpeak,r=-0.425,P〈0.01)、VO2peak/kg(r=-0.561,P〈0.001)、VAT(r=-0.726,P〈0.001)和左心室射血分数(LVEF,r=-0.264,P〈0.01)均呈显著负相关。多元逐步回归分析显示年龄(β=-0.283,P=0.005)和log(hs—CRP)(β=-0.516,P=0.000)是VO2peak降低的危险因素;年龄(β=-0.323,P=0.003)、体质指数(BMI,β=-0.265,P=0.014)和log(hs—CRP)(β=-0.547,P=0.000)是VO2peak/kg降低的危险因素;年龄(β=-0.206,P=0.048)和log(hs-CRP)(β=-0.515,P=0.000)是VAT降低的危险因素,而其中log(hs-CRP)是CRF降低的最主要危险因素之一。结论心血管疾病患者的血清hs—CRP与CRF、LVEF呈负相关性。血清hs—CRP可能是反映CRF及左心室收缩功能的有效标记物。
