多层螺旋CT对闭合性肠及肠系膜损伤的临床价值

Clinic value of MDCT in bowel and mesenteric closed injury

目的探讨多排螺旋CT对闭合性肠及肠系膜钝性损伤的临床价值。方法回顾性分析2007年1月～2009年2月经我院外科证实为肠及肠系膜损伤病人的CT表现、术前CT报告及手术记录。结果术前CT准确诊断肠及肠系膜损伤19例,1例漏诊,无误诊;肠及肠系膜CT表现为肠壁增厚9例、肠系膜渗出13例、游离气体11例、腹膜后及腹膜腔积液20例。结论肠及肠系膜损伤有其区别于腹腔实质性脏器的损伤CT表现,及时的CT检查对腹部闭合性外伤治疗方法的选择有重要的参考价值。

Objective To evaluate the clinic value of MDCT in bowel and mesenteric closed injury. Methods Twenty patients with abdomen closed injury which was proven by surgery were selected as the study objects. A retrospective analysis was done on the CT findings, preoperative CT reports and surgical records. Results Nineteen cases were diagnosed correctly by preoperative CT, one case was missed dignosis, and there weren＇t diagnostic errors;The CT features included bowel discontinuity（0 ease）, bowel-wall thickening（9 cases）, mesentery effusion （ 13 cases ）, extraluminal air（ 11 cases ）, intraperitoneal and retroperitoneal fluids （ 20 case）. Conclusion The CT finding of bowel and mesentery closed injury is different of solid organ injury, It have very important referenced value to perform abdomenal CT examination immediately after abdomen closed injury.
