经脐单孔腹腔镜胆囊切除术的临床应用与评价

Transumbilical single-port laparoscopic cholecystectomy:clinical practice and evaluation

目的:评价经脐单孔腹腔镜胆囊切除术(transumbilical single-port laparoscopic cholecystectomy,TUSPLC)的临床应用效果。方法:2009年6月至2010年2月为49例患者用常规器械行TUSPLC,观察手术可行性、安全性、微创性及美容效果。结果:48例(98%)成功,1例中转三孔腹腔镜胆囊切除术。手术时间平均53min,术中无并发症发生。术后发生切口血肿1例,积液2例。结论:TUSPLC技术上可行且相对安全,先行术中腹腔镜探查可有效降低手术中转率。切口并发症可能增加。疤痕得到较好隐藏是其主要优势。适于对美容效果有较高要求的患者。

Objective：To evaluate the clinical application effect of transumbilical single-port laparoscopic cholecystectomy（TUSPLC）.Methods：From Jun.2009 to Feb.2010,TUSPLC was performed in 49 patients using conventional instruments.The feasibility,safety,minimal invasiveness and cosmetic result of the technique were observed.Results：TUSPLC was completed in 48 patients（98%） with one conversion to three-port laparoscopic cholecystectomy.The mean operating time was 53min.No intraoperative complications happened.1 umbilical hematoma and 2 seroma occurred after operations.Conclusions：TUSPLC is feasible and safe.The conversion rate can be effectively decreased by laparoscopic exploration.The incisional complications may increase.The main advantage of the procedure is that the scar is well-concealed.TUSPLC should be indicated for patients with higher cosmetic demand.
