摘要
目的研究终未期肾脏疾病(ESRD)患者经血液透析治疗过程中机体急性时相反应物及氧化应激状态变化情况,对炎症反应和氧化应激之间的关系进行初步探讨。方法选取定期来凉山州第二人民医院门诊进行血透的ESRA患者27例,健康对照20例。分别检测其静脉血中超敏C-反应蛋白(hs-CRP)、脂蛋白a[-Lp(a)]和前清蛋白(PA)3种急性时相反应物及丙二醛(MDA)、总抗氧化态(TAS)和谷胱苷肽(GSH)3种氧化应激参数水平。结果检测数据经统计分析显示,ESRD患者体内急性时相反应物和氧化应激水平均存在异常(P均〈0.01),且这种异常不受患者性别因素影响(P均〉0.05);患者体内GSH水平同hs-CRP(r=0.43,P=0.025)和PA(r=0.436,P=0.023)水平存在明显相关;MDA水平同hs-CRP(r=0.638,P=0.000)水平存在明显相关。结论初步验证了ESRD患者机体中普遍存在微炎症和氧化应激状态,且二者存在相互关联。
Objective To investigate the change of acute-phase reactants and oxidative stress in patients with end-stage renal disease (ESRD). Preliminary study the relation of inflammation and oxidative stress in ESRD. Methods In the study, 27 ESRD patients on maintenance hemodialysis in hospital out-patient and 20 health controls were enrolled and measured the levels of 3 acute-phase reactants,including super-sensitivity C-reactive protein (hs-CRP),tipoprotein a [Lp (a)] and prealbumin (PA),and 3 oxidative stress parameters,including malondialdehyde (MDA),total anti-oxidation state (TAS) and gIutathione (GSH) in serum. Results The statistical results suggested that the level of acute-phase reactants and oxidative stress in ESRD patients were abnormal(P〈0. 01 for all). There were no association of sex with the levels of oxidative stress and acute-phase reactants (P〉0. 05 for all). Plasma level of GSH significantly correlated with hs-CRP(r= 0. 43, P = 0. 025) and PA(r= 0.436,P= 0. 023) and plasma level of M DA significantly correlated with hS-cRP(r= 0. 638,P= 0. 000). Conclusion It suggested that the microinflammation and oxidative stress in ESRD patients were general, and there was association of the levels of acute-phase reactants with oxidative stress.
