新生儿败血症耳垂血、指尖血白细胞计数结果分析

Significant Differences between Earlobe and Fingertip White Blood Counts in the Neonatal Period Predict Suspected Septicaemia

摘要 目的 比较新生儿患者耳垂血和指尖血血细胞计数的差异.方法 42例新生儿患者同时采集耳垂血和指尖血,并分析其全血细胞计数测定值的差异.结果 耳垂血WBC,RBC,HGB,HCT和MON%结果高于指尖血(P〈0.01).WBC高出的程度最为明显,若以高出20%为炎症组的诊断指标,该临界值的敏感度、特异度、准确度、PPV,NPV分别为90.5%,92.6%,91.4%,95.0%,86.7%.其它项目差异均无统计学显著性意义.结论 新生儿耳垂血WBC,RBC,HGB和HCT结果明显高于指尖血,临床需要校正后使用.WBC差异超过20%可能提示新生儿感染. Objective To assess the accuracy and clinical features of neonatal blood count in earlobe and fingertip. Methods Blood samples of earlobe and fingertip from 42 neonatal patients were collected and complete blood cell counts were measured and compared. Results There demonstrated a significant higher in WBC,RBC,HGB,HCT and MON % in ear- lobe blood samples than those in fingertip blood samples(P〈0. 001). WBC was of the highest difference. When using 20% difference of WBC between in earlobe and in fingertip blood samples as the cut-off ratio for infant infection diagnosis ,found out that 90. 5%,92.6 0%, 91.4 %, 95.0% and 86.7 % were respectively of the Sensitivity, Specificity, Accuracy, PPV and NPV. In other blood parameters,there were no significant differences. Conclusion The WBC,RBC,HGB and HCT in ear- lobe blood in neonates were higher than those in fingertip blood and need to be corrected in clinical practice. The difference of WBC between the two capillary methods predicts infection in neonates when it is more than 20%.
