肥胖型高血压患者血中脂联素、TNF-α、IL-6的测定及意义

Determination and value of serum adiponectin,TNF-a and IL-6 in patients with essential hypertension and obesity

目的:探讨肥胖型高血压患者血清脂联素、TNF-a、IL-6等指标含量改变并分析其相关性。方法:将97例高血压患者和96例对照者按体重指数分为正常对照组,单纯性超重/肥胖对照组,非超重/肥胖型高血压组、超重/肥胖型高血压组四个亚组,检测其血中脂联素、TNF-α、IL-6等指标。结果:与正常对照组相比较,高血压组FBG、TNF-α、IL-6出现明显升高而脂联素、HDL-C出现明显下降(均P<0.01)。超重/肥胖组与非肥胖组比较,脂联素有出现明显下降(P<0.01),而TNF-α、IL-6无显著差别(均P>0.05)。脂联素含量分别与TG、FBG、TNF-a、IL-6呈负相关(r分别为-0.464,-0.308、-0.357、-0.480,P<0.01)。TNF-a与IL-6呈正相关(r为0.340,P<0.01)。结论:脂联素下降与超重/肥胖之间存在一定的相关性,脂联素下降可引起糖及脂肪代谢障碍;脂联素降低、TNF-α、IL-6升高可能共同参与高血压的发病过程。

Objective：To study the level changes and the correlation among adiponectin,TNF-a and IL-6 in patients with essential hypertension and obesity.Methods：Ninety seven hypertensive patients and 96 controls were divided into four subgroups（normal control group,simple overweight/obesity group,non-overweight/obesity hypertension group,overweight/obesity hypertension group） according to their BMI.The concentrations of adiponectin,TNF-αand IL-6 in blood were determined.Results：Compared to normal control group,the levels of FBG,TNF-α and IL-6 were increased while both adiponectin and HDL-C were significantly reduced in hypertension groups（P〈0.01）.Compared with non-overweight/obesity group,adiponectin was significantly reduced（P〈0.01）,but TNF-αand IL-6 were not obviously increased（P〉0.05） in overweight/obesity groups.Adiponectin was negatively correlated with TG,FBG,TNF-a and IL-6 respectively（r=0.464,-0.308,-0.357,-0.480,P〈0.01）.TNF-a was positively correlated with IL-6（r=0.340,P〈0.01）.Conclusion：Decline in adiponectin was correlated with overweight and obesity which may induce disorders of metabolism of glucose and lipid.Both decrease of adiponectin,and increase of TNF-α and IL-6 may be involved in the pathogenesis of hypertension.
