6%羟乙基淀粉130/0.4对不同血型子宫肌瘤切除患者凝血功能的影响 被引量:1

摘要 目的观察6%羟乙基淀粉130/0.4急性高容量血液稀释(AHH)对不同血型的子宫肌瘤切除患者凝血功能的影响。方法选择ASAⅠ或Ⅱ级择期妇科手术患者60例,按血型分为O型组(O组)、A型组(A组)、B型组(B组)、AB型组(AB组)。全麻诱导后快速输注6%羟乙基淀粉130/0.4 20 ml/kg。术中连续监测MAP、HR和尿量;分别于AHH前(T0)、AHH后20 min(T1)、40min(T2)、80 min(T3)采集静脉血进行凝血活酶原时间(PT)、活化部分凝血活酶时间(APTT)、Wille-brand因子(v WF)、Ⅷ因子活动度(FⅧ:C)、纤维蛋白原(FIB)、血小板膜糖蛋白(GpⅡb/Ⅲa)和血栓弹力描记图(TEG)的测定。结果四组患者AHH前后血流动力学稳定。T1、T2时O组APTT均长于A、B、AB组(P<0.05)。T1~T3时四组v WF、FⅧ:C、FIB、GpⅡb/Ⅲa均明显低于T0时(P<0.01);T1、T2时O组患者v WF、FⅧ:C和GpⅡb/Ⅲa活性显著低于A、B、AB组(P<0.01)。四组患者TGE的凝血时间(K)和反应时间(R)均有轻微延长,T1~T3时O组а角小于A、B、AB组,但四组а角都在正常范围。结论 6%羟乙基淀粉130/0.4急性高容量血液稀释对O型血子宫肌瘤切除患者凝血功能的影响大于其他血型组。
