摘要
目的探讨磁共振检查对新生儿窒息后缺氧缺血性脑病(HIE)的诊断价值。方法对102例窒息新生儿在生后1周内进行头颅磁共振检查。结果重度窒息组HIE的病灶检出率为90%,轻度窒息组的病灶检出率为58.5%,重度组病灶检出率明显高于轻度组(P<0.01);早产儿HIE病灶检出率为68.1%,足月儿HIE的病灶检出率为61.8%,二者间无明显差异(P>0.05);DWI对缺血灶检出较敏感,SWI对出血灶检出较敏感。结论磁共振检查对HIE的诊断提供了准确的影像学依据,DWI联合SWI扫描可提高对HIE病灶的检出率。
Objective To investigate the diagnostic value of MRI on hypoxic ischemic encephalopathy caused by neonatal asphyxia. Method To detect 102 cases of asphyctic newborns' brain using MRI within one week after birth. Results The detection rate of lesion in severe asphyxia group was 90%, and mild asphyxia group was 58.5%. The detection rate of lesion in severe group was significantly higher than that in mild group (P〈0.01) . The detection rate of lesion in preterm infant was 68.1%, and in term infant was 61.8%. There was no significant difference between the two (P〉0.05) . DWI was more sensitive on ischemic focus, and SWI was sensitive on hemorrhagic focus. Conclusion MRI provides the accurate imaging for diagnosis of HIE, and DWI combined SWI can increased the detection rate of HIE.
