摘要
目的:通过事前干预不合理处方,提高门诊合理用药水平。方法:建立药学服务友情提示卡,就药师审方时发现的不合理内容与医师进行处方调配前沟通;并对我院门诊2007年5月~2009年5月不合理处方进行干预。结果与结论:不合理处方主要表现在剂量、用法不正确,选用剂型与给药途径不合理,未注明过敏试验及结果的判定等。应用药学服务友情提示卡对不合理处方进行干预,使干预门诊处方存在问题的关口前移,患者用药的安全性显著提高。
OBJECTIVE: To perform intervention on irrational prescriptions beforehand and to improve rational use of drugs. METHODS: Pharmaceutical service cards were set up to interfere in irrational out-patient prescriptions from May 2007 to May 2009. Pharmacists who found unreasonable content of prescription communicated with physicians before dispensing medicine. RESULTS CONCLUSIONS: Irrational prescriptions mainly manifest as wrong dosage and usage, unreasonable dosage form and administration route, unmarked sensitivity test and its result, etc. The problems of irrational out-patient prescriptions are found much earlier through intervention by pharmaceutical services cards to improve the safety of drug use in patients significantly.
