三级综合医院护理安全管理指标体系的研究

Study on nursing safety management index system for the first-class general hospital

目的研制一套适合医院内部使用的护理安全管理指标体系。方法在科学界定护理安全相关概念的基础上,通过文献回顾和安全管理理论分析,按照层次分析法,以Vincent医学框架为基础,识别影响护理安全的各级因素,通过小范围专家讨论认证,建立指标体系初步方案。采用德尔斐(Delphi)法组织两轮专家函询,汇总专家意见,确定指标命名和权重,建立护理安全管理指标体系。结果研制了一套由组织管理因素、背景环境因素、护理人员因素、患者因素、陪伴因素5项一级指标、13项二级指标和50项三级指标构成的护理安全管理指标体系。结论建立护理安全管理指标体系,确立了医院内部影响护理安全的各级因素,体现了系统管理的安全理念,优化了护理安全管理模式,为建立患者安全保障体系提供了管理工具,也为今后构建"以患者安全为核心"的医院护理质量评审标准提供了内容依据和量化标准。

Objective To establish a suitable nursing safety management index system for the first-class general hospital.Methods Through literature review,the authors analyzed the concepts involved in nursing safety management and organiza-tional accident mode,distinguished the influence factors of nursing safety according to the Vincent framework,and established the primary index system by discussion of a small scale of specialists and analysis of hierarchy process.Then Delphi method was taken to organize two rounds of specialist consultations,specialists＇ opinions were summarized,index name and weight were identified,and the nursing safety management index system was established.Results A nursing safety management index sys-tem for the first-class general hospital was established,which included five first-level indicators：organization and management, environment and background,nursing staff,patient,and accompanying status,13 second-level indicators,and 50 third-level indi-cators.Conclusion The index system involves the factors influencing nursing safety in first-class general hospital and reflects the concept of systematization management,which is of great significance to the guidance of current nursing safety management.The index system will provide evidences for nursing quality evaluation indicators.
