经阴子宫切除与腹腔镜辅助阴切的临床疗效比较

Clinical outcome of vaginal total hysterectomy and laparoscopy-assisted vaginal hysterectomy:A comparative study

目的比较阴式子宫全切除术和腹腔镜辅助经阴子宫切除术的临床效果。方法收集我院2008年9月-2010年3月间行子宫切除的203例患者,其中阴式子宫全切除术132例(TVH组),腹腔镜子宫全切除术71例(LAVH组)。比较两组疾病种类、手术时间、术中出血量、住院时间及术后恢复等。结果两组在手术时间、出血量、切除子宫大小方面,差异均有统计学意义(P<0.05);在术后恢复、住院时间方面两者差异无统计学意义。结论对有盆腔粘连、子宫较大的病例,可优先选择腹腔镜辅助阴切,可减少并发症的发生。

Objective To compare the clinical outcome of vaginal total hysterectomy（VTH） and laparoscop-assisted vaginal hysterectomy（LAVH）.Methods Types of disease,operation time,intraoperative bleeding,hospital stay time and postoperative recovery were compared in 203 women who underwent hysterectomy from September 2008 to March 2010 in our department.Of these patients,132 underwent VTH and 71 LAVH.Results No significant difference was found in operation time,blood loss,and resected uterus size between the two groups of patients undergoing VTH and LAVH,respectively（P0.05）,but a significant difference was observed in postoperative recovery and hospital stay time between the two groups of patients.Conclusion LAVH can be performed in those with pelvic adhesion or with a large uterus in order to reduce complications.
