论TCT技术在检测胸腹水中恶性肿瘤细胞的应用

摘要 目的:探讨膜式液基薄层细胞学技术(ThinPrep cytology test,TCT) 在检测胸腹水中恶性肿瘤细胞的应用价值.方法:将103例胸腹水标本分别采用传统普通细胞涂片法,和液基细胞学检查,比较两组整体制片质量及细胞学诊断结果.结果:采用TCT技术制片与传统涂片相比细胞多且集中,薄层均匀,背景干净.传统普通细胞涂片法确诊癌11例(10.6%),其中类型难定3例(27.3%) 可疑癌21例(20.3%) 阴性68例(66.0%).液基细胞学检查确诊癌29例(28.2%),其中类型难定仅1例(3.8%) 可疑癌8例(7.8%) 阴性65例(63.1%).两组阳性率比较差异有显著性(P〈0.05).结论:TCT技术与传统涂片相比在细胞标本的采集、制片和诊断技术方面都有很大进步,使胸腹水恶性肿瘤的确诊率明显提高.更能为临床治疗提供可靠的诊断依据. Objective:Discussion film type fluid base thin layer cytology technology (ThinPrep cytology test,TCT) in examination chest ascites malignant tumor cell application value. Methods: This uses the traditional ordinary cell smear law separately 103 example chest and belly water gauge, with fluid basal cell study inspection, compared with two group of overall movie--making qualities and cytology diagnosis result. Results: Uses the TCT technology movie--making and the traditional smear compares the cell to be many also to be centralized,the thin layer is even,the background is clean. The traditional ordinary cell smear law diagnosis cancer 11 examples (10. 6%) ,in which type difficult to decide 3 examples (27.3%) ;Suspicious cancer 21 examples (20.3%) ; Negative 68 examples (66.0%). The fluid basal cell study inspection diagnosis cancer 29 examples (28.2 %) ,in which type difficult to decide the only 1 example (3.8 %) ;Suspicio us cancer 8 examples (7.8%) ; Negative 65 examples (63.1%). Two group of masculine gender rate comparison difference has the significance (P〈0.05). Conclusion: The TCT technology and the traditional smear compare in cell specimen gathering, the movie-making and the diagnosis technology aspect all have are very greatly progressive, cause the chest ascites malignant tumor the diagnosis rate distinct enhancement. Can provide the reliable diagnosis basis for the clinical treatment.
