短节段固定结合单枚Cage治疗退变性腰椎滑脱症

Treatment of lumbar degenerative spondylolisthesis with short-segment fixation and mono-cage

目的探讨短节段固定结合单枚Cage治疗退变性腰椎滑脱症的临床疗效。方法对46例退变性腰椎滑脱症患者行后路减压和短节段椎弓根螺钉固定,椎间单枚Cage斜放植骨融合,定期摄X线片,观察Nakai评分及融合情况。结果 46例均获随访,时间6～92个月。患者腰背痛均有不同程度的改善,根据Nakai评分标准:优24例,良18例,可3例,差1例,优良率达91.3%。患者术后6～12个月均获得骨性融合。结论短节段椎弓根螺钉固定结合单枚Cage治疗退变性腰椎滑脱症,能获得理想复位、坚强内固定和可靠的融合,临床疗效满意。

Objective To observe the outcome of lumbar degenerative spondylolisthesis treated with short-segment fixation and mono-cage.Methods 46 cases of lumbar degenerative spondylolisthesis were treated with laminectomy,short-segment transpedicular screw rod fixation,and with single fusion cage inserted.All cases were followed up with X-ray films and Nakai scores.Results 46 patients were followed up for 6 to 92 months.The back pain improved at some extent in all patients.According to Nakai′s criteria for scoring,the results were excellent in 24 cases,good in 18 cases,fair in 3,and poor in 1.The excellent and good rate was 91.3%.All patients achieved successful radiographic fusion at 6 to 12 months.Conclusions For lumbar degenerative spondylolisthesis,satisfactory reduction and solid internal fixation and reliable fusion can be achieved by the short-segment pedicle screw fixation and mono-cage.The clinical effect of the treatment is excellent.
