社区2型糖尿病合并抑郁患者生活质量影响因素研究

Influencing Factors to Quality of Life in Patients with Type 2 Diabetes Mellitus Complicated by Depression

目的了解社区2型糖尿病患者抑郁的患病率及其生活质量的影响因素。方法采用整群抽样法,对北京市4个社区的2型糖尿病患者使用自评抑郁量表(SDS)进行抑郁筛查,再应用多元逐步回归法筛选对合并抑郁的T2DM患者生活质量有影响的因素。结果共调查619例2型糖尿病患者,其中273例合并抑郁(SDS得分≥53分),2型糖尿病患者中抑郁的患病率为44.10%。对273例2型糖尿病合并抑郁患者的生活质量影响因素进行多元逐步回归,有8个变量进入模型:SDS得分、有无并发症、社会支持评定量表(SSRS)主观支持维度得分、月收入、SSRS总分、治疗方法、文化程度、空腹血糖值;R2=36.5%。结论 2型糖尿病患者具有并发抑郁的高风险,合并抑郁的2型糖尿病患者其生活质量受到抑郁程度、并发症、社会支持状况、经济状况等因素的影响。

Objective To estimate the prevalence rate of depression in patients with Type 2 Diabetes Mellitus（DM）,and to explore the factors that have influence on quality of life （QOL） of the patients with Type 2 DM complicated by depression.Methods Cluster sampling was used to sample the patients with type 2 DM in four communities in Beijing city. Self-rating depression scale （SDS） was used to screen depression in DM patients,then the factors that have influence on QOL of the patients with Type 2 DM complicated by depression were analyzed by multivariate linear regression.Results Totally 619 patients with type 2 DM were investigated.Among them,273 （SDS score ≥53） were regarded as suffering depression,and the prevalence rate of depression in DM was 44.10%.Multiple stepwise regression analysis showed that there were eight statistics significant factors that had influence on QOL of the patients with type 2 DM complicated by depression,including SDS score,complication,score of subjective social support,monthly income,total score of social support rating scale,treatment,education background,fasting plasma glucose （FPG）;R2=36.5%.Conclusion Type 2 DM patients suffer high risk of complicated depression.The factors that have influence on QOL of the patients with type 2 DM complicated by depression include depression,complication,social support,and personal economic condition.
