“Y”形支架在治疗肝门部胆管梗阻中的应用

Application of "Y" Configuration Stents in Treatment of Hilar Obstructions

目的评价"Y"形支架治疗肝门部胆管梗阻的临床效果及价值。材料与方法12例肝门部胆管梗阻患者经皮肝穿刺,分别在左右肝管植入"Y"形支架,2枚支架并行放置10例,经第一枚支架网眼放置第2枚支架2例,其中8例联合抗肿瘤治疗。结果技术成功率100%,总胆红素术前(263.5±153.8)μmol/L,术后2周下降至(57.2±10.4)μmol/L,没有出现与植入支架有关的严重并发症发生及死亡病例。结论 "Y"形支架植入可有效解除肝门部胆管梗阻,联合抗肿瘤治疗可提高综合疗效。

Objective To evaluate the clinic effect and value of ＂Y＂ configuration stents in treatment of hilar obstructions. Methods ＂Y＂ configuration stents were placed percutaneously between right-left intrahepatic duct in 12 patients with hilar obstructions,two stents were placed parallelly in 10 patients,the second stent was placed through the mesh of the fi rst stent in 2 patients,and combined with anti-tumor therapy in 8 patients. Results The technical success rate was 100%. The level of Total Bilirubin was decreased from（263.5±153.8） μmol/L to（57.2±10.4） μmol/L after operation in two weeks,and no severe complications and decease was found. Conclusion The placement of ＂Y＂ configuration stents could relieve the hilar obstructions effectively,if combined with anti-tumor therapy it would improve the curative effect.
