摘要 目的:研究TSPOT-TB试剂盒在结核病临床诊断中的应用价值。方法:应用TSPOT-TB试剂盒检测结核感染组(包括26例临床已确诊结核病患者和4例PPD强阳性志愿者)和非结核感染组(包括20例PPD阴性的非结核病患者和10例PPD阴性的健康对照者)外周血单个核细胞,观察斑点形成细胞(SFC)的数量。结果:30例结核感染组中,28例(93.3%)TSPOT-TB检测阳性,其中4例PPD强阳性志愿者检测均为阳性;30例非结核感染组中,仅1例(3.4%)TSPOT-TB检测阳性。TSPOT-TB阳性组的SFC显著高于TSPOT-TB阴性组。在29例TSPOT-TB阳性者中,31.0%检测孔A(ESAT-6抗原)和B(CFP-10抗原)均阳性,44.8%仅检测孔A阳性,24.2%仅检测孔B阳性。结论:TSPOT-TB试剂盒在检测结核感染、辅助结核病诊断方面具有很高的应用价值。 Objective To investigate the diagnostic value of T SPOT-TB kit for tuberculosis. Methods The peripheral blood mononuclear cells (PBMC) from TB infection group (containing 26 TB patients and 4 volunteers with positive PPD skin test) and non-TB infection group (containing 20 non-TB patients and 10 healthy volunteers with negative PPD skin test) were detected by T SPOT-TB kit manufactured by Oxford Immunotec Inc., UK. The spot forming cells (SFC) were enumerated. Results In TB infection group, 93.3% (28 / 30) were positive by T SPOT-TB detection, and 4 volunteers with positive PPD skin test were all positive by T SPOT-TB detection. In non-TB infection group, only 1 (3.4%) was positive by T SPOT-TB detection. The SFCs in patients with positive T SPOT-TB detection were significantly higher than that in patients without positive T SPOT-TB detection. Of 29 cases with positive T SPOT-TB detection, 31.0% ESAT-6 peptide antigen and CFP-10 peptide antigen were all positive, 44.8% only ESAT-6 peptide antigen was positive, 24.2% only CFP-10 peptide antigen was positive. Conclusion The T SPOT-TB kit could be used to detect TB infection and to assist the diagnosis of tuberculosis.
