乳腺无肿物乳头溢液102例临床分析

A CLINICAL ANALYSIS OF 102 CASES OF NIPPLE DISCHARGE WITHOUT A LUMP IN THE BREAST

目的探讨乳腺无肿物乳头溢液与早期乳腺癌及癌前病变的关系。方法乳腺无肿物乳头溢液的102例患者，均经手术切除病变导管，做病理检验，如为恶性，则用乳腺癌改良根治术。结果本组病例良性者95例，占93．1％，其中乳腺导管内乳头状瘤和乳腺囊性增生病80例（78．4％），乳头状瘤（病）伴瘤细胞或导管上皮增生活跃（癌前病变）12例（11．7％），3例为慢性炎症（其中1例伴导管内异物）；T0期乳腺癌7例（6．9％），均为血性乳头溢液，占血性溢液的11．3％。结论对乳腺无肿物乳头溢液行病变导管切除，病理检验对检出早期乳腺癌及癌前病变有重要的临床价值。

Objective To explore the relationship between the discharge without a lump in the breast and the early mammary cancer and precancerous lesions. Methods 102 patients with nipple discharge without a lump in the same breast were treated by removing the diseased ducts. All the patients received ductectomy or local excision at first, and modified radical mastectomy would be performed if the tumor proved to be malignant after the pathological examination. Results in the treatment group, 95 cases (93.1% ) belonged to benign lesions. Among them, 80(78. 4 % ) suffered from intraductal papilloma and cystic hyperplasia, 12 (11. 7% ) from papilloma or papillomatosis with active epithelial hyperplasia, 7 (6. 9% ) mammary cancer at T0 stage showing bloody nipple discharge. The incidence of carcinoma associated with bloody discharge was 11. 3%. Conclusion Ductectomy and pathological examination in the treatment of nipple discharge without a lump in the breast has important clinical value in the detection of early mammary cancer and precancerous lesions.
