北京市西城区登记肺结核并发糖尿病流行病学特征

Epidemiological Characteristics of Tuberculosis Complicated with Diabetes in Xicheng District of Beijing City

目的了解肺结核患者并发糖尿病的流行病学趋势及发病特征,为制定相应防治策略提供依据。方法研究对象为2003年至2007年北京市西城区新登记肺结核患者。获得并发糖尿病肺结核患者的构成比及发病特点。结果并发糖尿病肺结核患者占患者总数的构成比从2003年的6.3%上升至2007年的11.1%。并发比例随着年龄的增大而增加,60岁以上年龄组达25.4%,特别60岁以上肺结核患者女性并发糖尿病的比例高达38.0%。与一般患者比较,并发糖尿病患者的痰涂片阳性率高达52.7%,患者中有空洞的比例也高达40.9%,均存在显著性差异。并发糖尿病涂阳肺结核患者治疗第2、5、6个月痰菌阴转率低于一般涂阳肺结核患者。结论糖尿病对结核病疫情影响显著,是目前结核病控制工作的重点,应加强对糖尿病患者的肺结核筛查,和对肺结核患者的糖尿病的诊断,特别是老年糖尿病患者,开展针对性的化疗研究,提高治愈率。

Objective To understand the epidemiological trend and the characteristics of tuberculosis patients with diabetes,in order to provide bases for making prevention strategy.Methods The objects of the study are the tuberculosis patients newly registered in Xicheng District,Beijing City during 2003 to 2007.To acquire the composition rate and the disease characteristics of tuberculosis patients complicated with diabetes.Results The composition rate of tuberculosis patients complicated with diabetes accounted for the total number of tuberculosis patients increased from 6.3% to 11.1%.Concurrent rate increased with age.The rate in over 60 age group reached 25.4%.Particularly,the rate in women over 60 years of tuberculosis patients complicated with diabetes reached 38.0%.Compared with normal patients,the rate of diabetic patients complicated with sputum smear-positive was 52.7%.The proportion of patients with tuberculous cavity reached 40.9%.The results were significantly different.Sputum conversion rates of tuberculosis patients complicated with diabetes and smear-positive are lower than normal smear-positive TB patient after 2 months,5 months and 6 months treatment.Conclusion The diabetes have significant affect on tuberculosis epidemic,and becomes the focus of TB control.We should enhance screening for tuberculosis in diabetic patients as well as the diagnosis of pulmonary tuberculosis in diabetic patients,especially in elderly patients group.We must to carry out targeted chemotherapy to improve cure rate.
