肝脏炎性肌纤维母细胞瘤CT诊断

CT Diagnosis of Liver Inflammatory Myofibroblastic Tumor

目的探讨肝脏炎性肌纤维母细胞瘤(inflammatory myofibroblastic tumor,IMT)的CT表现,以提高对本病的认识。资料与方法 9例经手术或穿刺活检病理证实的肝脏IMT均行CT平扫和增强多期扫描。结果 9例共10个病灶,平扫时9个病灶表现为低或稍低密度,增强动脉期8个病灶表现为轻度或不强化,门静脉期病灶进一步强化,8个病灶表现为轻度、中度强化,延迟期6个病灶表现为延迟强化。8个病灶出现周边环状强化或分隔状强化,3个呈"双环征"或"三环征",2个为"花瓣"状强化;10个病灶门静脉期均表现为边界清晰,5个有占位效应;动脉期显示灶周水肿4例,4例均伴发胆管扩张、结石。结论肝脏IMTCT表现具有一定的特征性,分析其CT增强特征可与肝内其他病变鉴别。

Objective To analyze CT manifestations of liver inflammatory myofibroblastic tumor and to improve the diagnostic accuracy. Materials and Methods All of the 9 cases which were comfirmed by surgery or needle biopsy were undergone plain CT scanning and enhanced scanning. Results Totally 10 lesions were found in 9 patients. 9 lesions showed slightly low or low-density on CT plain scanning.8 cases showed mild enhanced or not enhanced in arterial phase.Lesions further enhanced in portal venous phase and 8 lesions showed mild to moderate enhanced.6 lesions showed delayed enhancement in delayed phase.8 lesions showed peripheral ring-enhanced or separation-like enhanced.3 lesions showed double-ring sign or three-ring sign and 2 lesions showed petal-like enhanced.10 lesions appeared as a clear boundary in portal venous phase and 5 lesions with space-occupying effect.4 cases showed surrounding edema in arterial phase or with cholangiectasis or cholelithiasis. Conclusion Liver inflammatory myofibroblastic tumor has a certain characteristic manifestations on CT scanning.It can be identified with the other diseases of the liver by careful analysis of the enhanced CT features.
