摘要
目的研究中重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者经过外科手术治疗后,患者生活质量的变化。方法选择外科手术治疗中重度OSAHS患者40例,采用Calgary生活质量指数(Calgary sleep apnea quality of life index,SAQLI)在术前和术后6个月进行问卷调查,同时术前、术后6个月填写Epworth嗜睡量表(Epworth sleepiness score,ESS),并且复查多道睡眠图(PSG)监测。结果术后6个月复查PSG与治疗前相比,呼吸暂停指数和夜间最低动脉血氧饱和度明显改善(P<0.05);SAQLI总分从术前3.91±0.5提高到术后4.99±1.1,具有显著性的统计学意义,其中以日常活动、症状方面更明显,社会活动及情感改善不明显,嗜睡症状明显好转。结论对OSAHS患者合理治疗,可有效提高生活质量;将ESS和SAQLI联合使用筛选高危患者是一个经济、简便和有效的方法。
OBJECTIVE The aim of this study wasto explore the effects of surgical treatment on health-related quality of life in patients with moderate and severe obstructive sleep apnea hypopnea syndrome (OSAHS). METHODS Forty patients with moderate and severe OSAHS received surgical treatment, and the Calgary sleep apnea quality of life index(SAQLI) questionnaires were completed before and 6 months after operation. Then health-related quality of life included daily functioning, social interactions, emotional functioning and symptoms. The total scale was the sum of the average scale of each domain. Also polysomnography was reexamined at 6 month after operation. RESULTS After surgical treatment, the apnea and hypopnea index and lowest arterial oxygen saturation was significantly changed(P 0.05). The mean score on SAQLI was 3.91±0.9 before operation, and the mean score on SAQLI increased to 4.99±1.1(P 0.05) after operation, especially the aspects of daily functioning and symptoms were improved significantly. The Epworth sleepiness scale(ESS)was improved significantly. CONCLUSION Health-related quality of life in patients with OSAHS is improved effectively after surgical treatment. Maybe ESS plus SAQLI is an economical, simple, and effective screening method for suspected patients.
