探讨外伤性血气胸的首选检查和治疗的方法

The disscusion on method of early accurate diagnosis and primary treatment of traumatic hemopneumothorax

目的探讨外伤性血气胸的早期首选检查和首选治疗方法。方法回顾性分析56例外伤性血气胸的早期检查、早期诊断和首选治疗方法。结果早期作CT扫描49例,早期行胸膜腔闭式引流术治疗48例,其中1例转为剖胸止血术,1例转电视胸腔镜治疗;保守治疗12例,其中8例因血气胸加重而转行胸膜腔闭式引流术,3例行胸膜腔单次或多次穿刺术治疗,所有病例均治愈,无远期并发症。结论 CT扫描能灵敏、准确的诊断血气胸的量;早期胸腔闭式引流术是治疗外伤性血气胸的首选治疗方法。

Objective To explore the method of early accurate diagnosis and primary treatment of trau-matic hemopneumothorax. Methods Retrospective analysis on early examinations,early diagosis and primary treatments of traumatic hemopneumothorax about 56 cases . Results In early stage,49 cases were made CT scan,48 cases were performed with closed drainage of pleural cavity,among them,one turned to hemostasis with thoracotomy and another one with VATS treatment. 12 cases were taken conservative theray,however,8 of them got worse and had to make the closed drainage. In another 3 cases,one or more times of pleurocentesis was performed. All patients were cured and without long-term complications. Conclusion CT scan can make a sensitive accurate evaluation of hemopneumothorax. And early closed drainage of pleural cavity is the principal treatment method of traumatic hemopneumothorax.
