摘要
目的探讨体重指数(BMI)对慢性阻塞性肺疾病患者运动能力是否存在影响。方法选择2008年2月至2009年2月于我院住院的COPD患者45例,男性27例,女性18例,肺功能均为(COPDⅡ~Ⅳ级)。将BMI<18.5kg/m2分为A组;BMI18.5~23.9kg/m2分为B组;BMI24~27.9kg/m2分为C组。记录三组患者住院时的上臂肌周长、肱三头肌皮褶厚度、血气分析、肺功能、6分钟步行距离;治疗后复查三组患者的血气分析、肺功能、6分钟步行距离。结果发现A组患者住院时MAMC,TSF,FEV1,MVV及6MWT明显偏低,治疗后肺功能及6MWT改善不明显。结论低体重指数COPD的患者呼吸肌肌力及耐力下降,导致其通气功能下降,及活动耐力下降;由于骨骼肌的萎缩进一步限制了患者的活动能力。
Objective To observe the influence of body mass index(BMI)to the COPD patients' locomotivity.Methods 45 cases diagnosed as COPD and hospitalized in our department from February,2008 to February,2009 were chosen in our study(male 27,female 18).Pulmonary function:(COPD II-IV).Group A was defined as BMI of 18.5 Kg/m2,Group B as BMI of 18.5-23.9 Kg/m^2,Group C as BMI of 24-27.9 Kg/m^2.All the correlative information including MAMC,TSF,ABG,pulmonary function and 6MWT of those subjects were collected.After the treatment,ABG,pulmonary function and 6 MWT of these groups were checked.Results The patients of the Group A had obviously low value of MAMC,TSF,FEV1,MVV and 6 MWT.After therapy the improvement of pulmonary function and 6 MWT were not obvious.Conclusion COPD patients with low BMI have features as following:the myodynamia and stamina of respiratory muscle descend,which lead to the decrease of ventilation capacity as well as activity tolerance,the atrophy of bone muscle make more limitation to the patients' exercise tolerance.
