西酞普兰与氯咪帕明治疗抑郁症对照研究

A control study of citalopram vs. clomipramine in the treatment of depression

目的 评价西酞普兰与氯咪帕明治疗抑郁症的临床疗效和安全性. 方法 将64例抑郁症患者随机分为两组,每组32例,研究组口服西酞普兰治疗,对照组口服氯咪帕明治疗,观察6周.于治疗前及治疗1周、2周、4周、6周末采用汉密顿抑郁量表及临床病情严重程度量表评定临床疗效,副反应量表评定不良反应. 结果 治疗后两组汉密顿抑郁量表及临床病情严重程度量表评分均较治疗前有显著下降（P＜0.01）;研究组治疗第2周末汉密顿抑郁量表总分较对照组下降更显著（P＜0.01）,其他时点评分均无显著性差异（P＞0.05）.两组不良反应较轻微,但研究组副反应量表评分显著低于对照组（P＜0.05）.结论西酞普兰与氯咪帕明治疗抑郁症疗效均显著,总体疗效相当,但西酞普兰起效更快,控制抑郁症状迅速,安全性更高,依从性更好.

Objective To evaluate the clinical efficacy and safty of citalopram vs.clomipramine in the treatment of depression. Methods Sixty-foUr depression patients were randomly divided into two groups of 32 ones each, research group took orally citalopram and control group did clomipramine for 6 weeks. Clinical efficacies were assessed with the Hamilton Depression Scale （HAMD） and Clinical Global Impression-Severity of Illness （CGI-SI） and adverse reactions with the Treatment Emergent Symptom Scale （TESS） before treatment and at the end of the 1st, 2nd, 4th and 6th week. Results After treatment, the scores of the HAMD and CGI-SI of both groups lowered more significantly compared with pretreatment （P〈0.01） ; at the end of the 2nd week, the total score of the HAMD was significantly lower in the research than in the control group （P〈0.01）, but there was no striking difference at other points （P〉0.05）. Adverse reactions of both groups were mild, but the score of the TESS was significantly lower in the research than in the control group （P〈0.05）. Conclusion Both citapolam and clomipramine have a significant curative effect in depression and their general efficacies are equivalent, but citapolam takes effect more rapidly, controls depressive symptoms quickly, has higher safety and better compliance.
